Sample permit applications are provided as the prescribed form for use by local units of government. This form has been designed to be generic and only requires that the local unit of government provide a name and address at the top of the form. Fee information may also be inserted on the application or the fee schedule may be provided as a separate document. Completion of the entire application is mandatory to assure appropriate approvals and inspections.
