Welcome to the Quality Service Audit Checklist (QSAC) program. QSAC is a voluntary management standards program that assists public libraries by setting benchmarks for Governance & Administration, Human Resources, Services, Collection Development, Technology, Facilities & Equipment and Public Relations. The Library of Michigan certifies public libraries as they achieve each level. Libraries can be certified at the Essential, Enhanced or Excellent levels.
To work on or complete a level, you will need to click on the form you need and save it on your computer.

Instructions Using the Editable PDFs:
1.Open the PDF of the Level you wish to work on. Save the PDF locally. Fill in the Library Name at the top. Continue working through the form, saving as you go.
2. Start with the Essential Level, filling out the completion date for each item
in each of the six areas of service. You may use either the date when the measure was actually completed or the current date. Comments are optional. In the comments section, you must press enter to go from one line to the next.

Upon completion, sign the Certification Form at the end of the report and mail the report and form to the address listed on the form.
4. The Library of Michigan will send you a certificate verifying that your library has achieved Essential Level status. A sample press release for you to adapt for your local media can be found at the following link: QSAC Sample Press Release.

After you receive your Essential Level Certification from the Library of Michigan, you may begin the same procedure for the Enhanced Level. When you receive Enhanced Level certification, you may continue on to the Excellent Level.
